Vint,

My vague recollection is that the first name change occurred as a
byproduct of the 1973 Mansfield Amendment (although perhaps a bit
later). but that the name was changed back and forth once or twice
before the "D" settled in permanently.  A final name change somewhat
after 1977 would be consistent with that, but I''m reasonably sure
that the first one occurred earlier.  Should be that hard to track
down if it were really important.
