[Chapter-delegates] Reminder!: Update Chapter Records for Year-End Performance Evaluations

Dear Nick,

Hope you are doing well!
As you mention in your email, I should update chapter records via the
Chapter Portal (AMS) by 16 November.
Within the board, nobody has historical memory of how it was done in
previous years.
It would be very helpful if you let me know if we are missing some topics
for evaluation
