Fred,

for more precision, what is decided in the ICANN framework is who is 
delegated the responsibility to manage the ccTLD, and changes thereafter.

As stated by you and Veni, the two-letter string to identify the ccTLD is 
decided by the ISO 3166 Maintenance Agency. This one in turn first has to 
have a distinct economy identified by the UN Statistics Division. Both 
entities "larger" and "smaller" than what people usually identify as 
countries may be in that list.
