Two things: first, that is decided by ICANN, and second, ICANN looks  
at ISO 3166, and specifically the list that one would find the  
codes .fr, .de, .bg, etc in. Note that while we call them "country  
code top level domains", this list is actually not of countries nor  
making any comment on whether they are countries.

We can help them get in touch with the right people. Once there, it  
is between them and the people.
