[Chapter-delegates] Chapter Membership Cancellation Notifications

   Thank you, Todd.

   Just in case there's another glitch somewhere, it would be good if the 
Chapter AMS contact and/or Chapter membership officer receives a notification 
email whenever any chapter member is dropped from the chapter rolls for any reason.

   I think i've received only 1 notification of chapter membership cancellation 
recently, but our regional bureau has mentioned that about 20 have dropped due 
to privacy opt-out.
