[Chapter-delegates] Questionnaire/Survey for Internet Access and Affordability

I am looking to conduct a research on Internet Access and Affordability in
Tanzania. My target are citizen living in sub-urban and rural area. In case
you have standard questionnaire/survey questions please share to me to
avoid reverting the wheel.
