[Chapter-delegates] Application to form an ISOC Chapter in Canada (Central)

Hi,

I am familiar with some of the principals and have supported the
development of this application. I am happy to see the "Canada
(Central)" compromise in the Chapter title, recognizing ISOC Quebec.
