[Chapter-delegates] survey

Veni,

I used the Survey Monkey to re-create the survey and included it in 
the attached zip file to a message I sent directly to you.  I am not 
sure where to post the survey but I am also inclined to create the 
page and data collection options myself so I know all about what 
happened to provide the results.

Please let me know what you prefer and, if you want me to take it from 
here, please let me know what page to which to connect the survey or 
whether you prefer to continue to use Survey Monkey and use the email 
option at you did originally?
